1. What is personal data?

Personal data is, according to the RODO and the Decree, information about an identified or identifiable natural person. Such data are, for example, name, surname, email address. The aforementioned personal data may be published on the pages of our website and also collected during an individual transfer or provided by you via email, e.g. by sending a message to one of the two available parish email addresses.

2. Who is the data controller?

The administrator of the data is the Sanctuary of Our Lady of Licheń in Licheń Stary, Klasztorna 4, 62-563 Licheń Stary, tel.: 63 270 81 63; e-mail: lichen@lichen.pl, hereinafter referred to as "Sanctuary". Only in the case of payment operations carried out through the website, the controller of the data is the Religious House of the Congregation of Marian Fathers, which makes the data available to the Shrine.

Pursuant to Article 7 (1) (1) of the General Decree on the Protection of Individuals with regard to the Processing of Personal Data in the Catholic Church issued by the Polish Bishops' Conference on March 13, 2018, the controller of the data is the Religious House of the Congregation of Marian Fathers in Licheń, which also makes the data available to the Sanctuary of Our Lady of Licheń for the purpose of sending correspondence and formation materials issued by the Sanctuary of Our Lady of Licheń. The above consent was given voluntarily in accordance with Article 5, point 7) of the Decree and may be revoked at any time.

   4 Period of processing of personal data?

Personal data processed on the basis of expressed Article 6(1)(a) RODO and Article 7(1)(1). The data will be processed until you withdraw your consent.
Personal data of individuals for whom consent is not required are processed on the basis of Article 6(1)(e) and (f) of the RODO and Article 7(1)(5) and (6) of the Decree, i.e. for the purpose of a task carried out in the public interest, as well as the legitimate interest of the administrator. The data will be processed until an effective objection is made.
The personal data of individuals as to which are processed pursuant to Article 6(1)(c) of the RODO and Article 7(1)(3) of the Decree, i.e. for the purposes of the law. The data will be processed until the time required to be stored by these regulations.

   5 To whom we share your personal data?

In connection with the processing of data, they are disclosed to external entities, including in particular: suppliers responsible for the provision and operation of IT systems, entities providing legal, accounting, auditing services, processors. In addition, personal data may also be disclosed to competent authorities or third parties who request such information, based on the relevant legal basis and in accordance with the provisions of applicable law. Data may also be transferred to payment operators in the event that such a service is provided.

   6. What are your rights related to the processing of personal data?

You have the following rights in connection with our processing of your personal data:
(a) the right to access your personal data, including obtaining a copy of the data,
(b) The right to request rectification of personal data,
(c) the right to erasure of personal data,
(d) The right to object to the processing of personal data,
(e) The right to withdraw consent to the processing of personal data,
(f) the right to portability of personal data,
(g) The right to restrict data processing,

(i) In the case of data relating to matters of other non-belief, a complaint can be made to the President of the Personal Data Protection Authority. The current address of the supervisory authority is: President of the Office for Personal Data Protection, 2 Stawki Street, 00-193 Warsaw.

   7. transfer of data outside the European Economic Area?

The level of protection of personal data outside the European Economic Area (EEA) differs from that provided by European law. For this reason, the Administrator transfers personal data outside the EEA only when necessary and with an adequate level of protection, primarily by:
(a) cooperation with processors of personal data in countries for which a relevant decision of the European Commission has been issued;
(b) use of standard contractual clauses issued by the European Commission;
(c) application of binding corporate rules approved by the relevant supervisory authority;
(d) The controller always informs about the intention to transfer personal data outside the EEA at the stage of collection.

  8. Przetwarzanie danych w związku z funkcjonowaniem Muzeum im. ks. Józefa Jarzębowskiego w Licheniu

    8.1 Administrator danych osobowych

Administratorem danych osobowych przetwarzanych w związku z działalnością zarówno Sanktuarium Matki Bożej Licheńskiej w Licheniu Starym jak i z funkcjonowaniem Muzeum im. ks. Józefa Jarzębowskiego MIC w Licheniu Starym (zwanym dalej “Muzeum”) jest Sanktuarium Matki Bożej Licheńskiej w Licheniu Starym (zwane dalej: „Sanktuarium”). Jedynie w przypadku operacji płatniczych realizowanych przez stronę internetową, administratorem danych jest Dom zakonny Zgromadzenia Księży Marianów, który udostępnia te dane Sanktuarium.Inspektorem Ochrony Danych Osobowych wyznaczonym przez Sanktuarium jest ks.     8.2. Klauzule informacyjne dla formularzy umieszczonych na stronie Muzeum

      8.2.1 Klauzula informacyjna - formularz kontaktowy Muzeum

Na podstawie art. 13 RODO informujemy, że:
a) Pani/Pana dane (imię i nazwisko, adres e-mail, numer telefonu, treść wiadomości)
przetwarzane są w celu udzielenia odpowiedzi na zapytanie przesłane za pomocą
formularza kontaktowego,
b) podstawą prawną przetwarzania jest art. 6 ust. 1 lit. f) RODO - prawnie uzasadniony
interes Administratora (udzielenie odpowiedzi),
c) podanie danych jest dobrowolne, lecz niezbędne do obsługi zapytania; odmowa
podania uniemożliwi udzielenie odpowiedzi,
d) dane będą przechowywane przez okres niezbędny do udzielenia odpowiedzi, nie
dłużej jednak niż 2 lata od daty wysłania wiadomości (na cele archiwalne i
ewentualne reklamacje),
e) przysługuje Pani/Panu prawo dostępu, sprostowania, usunięcia, ograniczenia
przetwarzania oraz prawo wniesienia sprzeciwu. W razie naruszenia - prawo
wniesienia skargi do Prezesa UODO.
f) nie stosujemy zautomatyzowanego podejmowania decyzji ani profilowania wodniesieniu do danych zbieranych za pomocą tego formularza.

      8.2.2. Klauzula informacyjna - zamówienie lekcji muzealnej i oprowadzania po Muzeum

W związku z rezerwacją lekcji muzealnej lub oprowadzania możliwej do dokonania przez formularz zamówień informujemy, że:
a) zakres przetwarzanych danych to: imię i nazwisko nauczyciela/opiekuna, jego numer
telefonu, nazwa i adres szkoły/instytucji, liczba i wiek uczestników, preferowana data
i tematyka lekcji,
b) cele przetwarzania: przygotowanie i realizacja usługi edukacyjnej (lekcji muzealnej
lub oprowadzania),
c) podstawy prawne: art. 6 ust. 1 lit. b) RODO - wykonanie umowy/usługi; art. 6 ust. 1lit. c) - wypełnienie obowiązków prawnych ciążących na Administratorze,
d) podanie danych jest warunkiem zawarcia umowy - odmowa skutkuje niemożliwością
realizacji rezerwacji,
e) okres przechowywania: 5 lat od daty realizacji usługi (archiwizacja dokumentacji
edukacyjnej oraz cele rachunkowe zgodnie z obowiązującymi przepisami),
f) przysługujące prawa: dostęp, sprostowanie, usunięcie (po upływie okresu
archiwizacji), ograniczenie przetwarzania, sprzeciw, prawo do przenoszenia danych
oraz skarga do UODO.
g) nie stosujemy zautomatyzowanego podejmowania decyzji ani profilowania wodniesieniu do danych zbieranych za pomocą tego formularza.

      8.2.3. Klauzula informacyjna - newsletter Muzeum

Jeżeli wyraziła Pani/Pan zgodę na otrzymywanie newslettera Muzeum informujemy, że:
a) zakres przetwarzanych danych to: imię i nazwisko, adres e-mail,
b) cele przetwarzania: przesyłanie informacji związanych z Muzeum np. na temat
aktualności, wystaw, wydarzeń itp. w formie newslettera,
c) podstawy prawne: art. 6 ust. 1 lit. a) na podstawie udzielonej zgody;d) podanie danych jest dobrowolne; możliwe jest wycofanie zgody w każdej chwili
poprzez link „wypisz się” w stopce każdej wiadomości,
e) okres przetwarzania - do momentu wycofania zgody,f) przysługujące Pani/Panu prawa: wycofanie zgody, prawo dostępu, sprostowania,
usunięcia, ograniczenia przetwarzania, sprzeciw, prawo do skargi do UODO.
g) nie stosujemy zautomatyzowanego podejmowania decyzji ani profilowania wodniesieniu do danych zbieranych za pomocą tego formularza.

    8.3. Informacja o obowiązkach i dobrowolności danych:

Wszystkie pola oznaczone gwiazdką (*) w formularzach są obowiązkowe; dane nieoznaczone są zbierane w sposób dobrowolny. Konsekwencją niepodania wymaganych danych jest brak możliwości realizacji usługi lub udzielenia odpowiedzi.
